Instructions for Completing Application Online

Click on the application button to open the application.  The file will open in
another browser as an Excel Spreadsheet.  You then have 2 options:

  1. Save the Excel spreadsheet to your computer                                         
    (remember what folder you save it to)
  2. Fill out the application
  3. Save the file again once you have completed the application
  4. Click the email link on the application
  5. Attach the file from your computer to the email
                                                          
                                                             OR

  1. fill out the application online
  2. print the file
  3. fax it to secure eFax number on application or mail it to the address
